Social Work and Counseling Services Manager                   

Shift: Full Time Monday-Friday with administrative on-call rotation

The Social Work and Counseling Services Manager is responsible for supervising all aspects of Social Work, Grief Support, Spiritual Care and other supportive care programs.

  • Provides leadership in the development of organizational goals and the monitoring of related agency metrics.
  • Translates the organization’s strategic plan into specific individual and group behavioral expectations and objectives through improved work methods, processes, and procedures.
  • Participates in the development and monitoring of competency skills for hospice interdisciplinary team members.
  • Plans, organizes and coordinates the Hospice interdisciplinary team. Assures the coordination and appropriate utilization of the services of each hospice discipline.
  • Provides clinical supervision to patient care staff to assure quality standards are met in compliance with Medicare, the Joint Commission, and State regulations and standards.
  • Implements the standards for use of electronic medical records to meet agency, Federal, State and Joint Commission standards.
  • Participates in the development of agency budget and provides oversight of responsible areas.
  • Provides staff development and education and assists program staff to understand their work in relation to the total program of the organization.
  • Actively promotes and participates in organization-wide performance improvement activities.
  • Participates in internal and external meetings to inform board, staff, volunteers and stakeholders about the mission, services and programs of the organization.

Qualifications:

  • A Masters Degree from an accredited program in either social work or psychology required.
  • Three (3) years of supervisory experience in a Hospice and/or home/community health oriented environment is preferred, with progressively responsible experience in supervisory or administrative duties.
  • Maintains license/registration as either a Masters Social Worker or a Limited Licensed Psychologist from the State of Michigan.
  • Experience with hospice, palliative and end of life care preferred.
  • Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Social Worker                                  

Shift: 40 Hours Per Pay Period (Mason and Oceana counties), Days may vary

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Qualifications:

  • A graduate of a Master’s program in social work accredited by the Council on Social Work Education and holds current certification in the State of Michigan or has a Master’s degree in psychology, sociology, counseling, human services or other field related to Social Work who would be supervised by an LMSW.
  • Current State of Michigan Social Work License (LMSW or LLMSW).
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

CENA Home Care                        

Shift: 60 Hours Per Pay Period, 8:00 am – 4:00 pm (may vary dependent upon needs). Various On Call Weekends

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current state of Michigan Nurse Aide Certification with no findings of patient abuse.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

RN PRN Residence Nurse – Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

(1) Float Homecare Nurse – Registered Nurse                                                        

**Signing & Retention Bonus $3,000.00!**

Shift: 72 Hours Per Pay Period, M-F 8:00 am – 4:30 pm, Weekend Varies

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited school of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
