Current Job Postings

Registered Nurse Case Manager – Home Care RN            

Shift: 80 Hours Per Pay Period, 8:00 am – 4:30 pm.  Weekends vary.

The Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • Graduate of an approved Nursing education program.
  • Current state of Michigan Registered Nurse License.
  • Possesses and maintains a current CPR certification.
  • Certification in hospice and palliative care preferred.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Maintenance Technician (Poppen Hospice Residence) 

Shift: 18 Hours Per Pay Period, Hours and days could vary.

The Clinical Manager is responsible for maintaining the residence in good repair and ensuring all systems are in compliance with applicable safety and fire regulation and federal, state and local building codes to ensure a safe and comfortable environment. Performs other functions within the facility including safety checks and maintaining equipment.

Qualifications:

  • High School Graduate
  • At least two year’s general maintenance experience is required. Previous medical care facility experience preferred.
  • Understands and adheres to the hospice mission, policies and procedures while advocating this philosophy.
  • Language skills, math skills and reasoning ability are required.
  • The ability to work with little supervision, perform duties and responsibilities in a manner that exemplifies excellent customer service.
  • Cooperative and flexible in fulfilling role obligation and has ability to work within an interdisciplinary team.
  • Ability to maintain open and positive relations with vendors.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Social Worker – PRN                   

Shift: As needed (weekday)

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Qualifications:

  • A graduate of a Master’s program in social work accredited by the Council on Social Education and holds current State of Michigan certificaton (LMSW or LLMSW) OR has a Master’s degree in psychology, sociology, counseling, human services or other related field who would be supervised by an LMSW.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current State of Michigan Nurse Aide Certification
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed.

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
