PRN CENA Residence/Homecare                                                

Shift: This position varies

The PRN CENA under the direction and ongoing supervision of the registered nurse, provides essential personal assistance to patients.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current state of Michigan Nurse Aide Certification with no findings of patient abuse.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Intake RN – Registered Nurse                                                        

**Signing & Retention Bonus $3,000.00!**

Shift: 80 Hours Per Pay Period, 12:00 pm – 8:30 pm

The Intake RN is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.

Qualifications:

  • A graduate of an accredited school of Nursing.
  • Current state of Michigan Registered Nurse License.
  • BSN preferred with experience in community-based care.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Referral Liaison                                                      

Shift: 48 Hours Per Pay Period, plus every other weekend

The Referral Liaison Assistant assists the intake department in maintaining the smooth operations of the admissions functions of the agency and also provides clerical support to the administrative staff.

Qualifications:

  • Certified Medical Assistant preferred with six months to one year related experience or equivalent combination of education and healthcare experience.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Essential Functions and Duties:

  • Responsible for Information Systems data input—On-going use of EMR and Outlook.
  • Takes all incoming information, referral, and other calls as relates to intake/admission process.
  • Schedules visits with prospective patients and intake staff.
  • Medical knowledge to secure verbal or written certification of terminal illness.
  • Appropriately request medical documentation from referral sources as needed.
  • Acts as liaison to DME, pharmacy, etc. as needed.
  • Provides clerical support to the administrative staff including but not limited to medical records and medical director.
