Current Job Postings

Referral Liaison

Shift: 48 Hours Per Pay Period (Schedule varies)

The Referral Liaison assists the intake department in maintaining the smooth operations of the admissions functions of the agency.

Qualifications:

  • Associates degree (A.A.) or equivalent from two-year college or technical school; or sic months to one year related experience and/or training; or equivalent combination of education and experience.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Social Worker

Shift: 64 Hours Per Pay Period, M-F 8:00 am – 4:30 pm (Days and hours may vary)

Provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Qualifications:

  • A graduate of a Master’s program in social work accredited by the Council on Social Work and holds current certification in the State of Michigan or has a Master’s degree in psychology, sociology, counseling, human services or other field related to Social Work who would be supervised by an LMSW.
  • Current State of Michigan Social Work License (LMSW or LLMSW).
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Social Work & Counseling Services Manager

Shift: 80 Hours Per Pay Period, 8:00 am – 5:00 pm (Days and hours may vary)

The Social Work and Counseling Services Manager is responsible for supervising all aspects of Social Work, Grief Support, Spiritual Care and other supportive care programs.  The manager ensures both quality of services and appropriate quantity of services to best meet client and family needs.  He/or she considers the organization’s mission and the organization’s business needs in all decision making.

Qualifications:

  • A Masters Degree from an accredited program in either social work or psychology required.
  • Three (3) years of supervisory experience in a hospice and/or home/community health oriented environment is preferred, with progressively responsible experience in supervisory or administrative duties.
  • Maintains license/registration as either a Masters Social Worker or a Limited Licensed Psychologist from the State of Michigan.
  • Experience with hospice, palliative and en of life care preferred.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Dietary Aide

Shift: 64 Hours Per Pay Period

Responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans. Ensure a safe clean environment by providing housekeeping and laundry services.

 

Qualifications:

  • High School Graduate.
  • General dietary experience is preferred.
  • Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Home Care Float RN – Registered Nurse (Two open positions)

Shift: 72 Hours Per Pay Period, M-F 8:00 am – 4:30 pm (varies) and 8:00 am – 4:30 pm Every Other Weekend

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

Qualifications:

  • Is a graduate of an accredited school of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

PRN Residence Nurse – Registered Nurse

Shift: Varies

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

Qualifications:

  • Is a graduate of an accredited school of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
  • Must be very flexible and able to work all shifts.
  • Able to work various roles at our hospice residence.

Residence CENA

Shift: Every Other Weekend 3 pm – 11:30 pm and 11:00 pm – 7:30 am

Provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current state of Michigan Nurse Aid Certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Home Care CENA PRN

Shift: Part Time

Provides direct care to patients in Home Care and/or the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current state of Michigan Nurse Aid Certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
