At Home
Home care is the core of hospice care, and Harbor Hospice understands the importance for so many of being at home surrounded by the people, the pets, and the lifetime of things they love. Home is where memories are made. Being cared for by loved ones in those familiar surroundings can bring immense comfort and peace to a person, especially as they near the end of their life. Harbor Hospice can help make it possible for a person to remain in their home during that tender time, with the support of family members, friends, and caregivers.
We also recognize it is not an easy task for a family to take on the role of caregiver for a loved one who nearing the end of their life. The Harbor Hospice team will teach family members how to care for their loved one and will provide care support with visiting nurses, physicians, social workers, certified home health aides, and others with 24-hour on-call contact.

Harbor Hospice provides care anywhere a patient lives, including in hospitals, nursing homes, and assisted-living facilities. The Harbor Hospice team shares their expertise in end-of-life care with the center staff so patients can have the best life possible at the end of their lives.
