Hospice Physicians and Nurse Practitioners

These key members of our medical team work with patients, family members, and caregivers to oversee patient hospice care, create a plan that anticipates patient needs, and address new concerns as they arise so the patient’s final days can be as peaceful, comfortable, and meaningful as possible.

Registered Nurses

Specially trained hospice nurses make regular visits with patients and are on call 24 hours a day, so you always have the support and assurance you need. They educate patients, families, and caregivers and continually monitor the patient’s condition so they can quickly meet emerging physical and emotional needs.

Certified Nursing Assistants (CNAs) and Home Health Aides

When patients need assistance with tasks such as bathing, washing their hair and brushing their teeth, caring for simple wounds, toileting and eating, our CNAs and home health aides will provide regular support to lift those activities from the shoulders of family members and caregivers. Additional services such as washing a patient’s laundry and preparing meals are also available.

Social Workers and Grief Support Counselors

Family members and caregivers often need help with their own struggles, such as figuring out how to say goodbye and what to do when they are overwhelmed with grief. Our social workers and counselors offer emotional, social, financial, and spiritual assistance. Grief counseling is available to any member of the community, regardless of whether they have been involved with Harbor Hospice. 

We offer several avenues to help people find peace as they grieve, including in-person or phone conversations with a counselor, group sessions for people who are drawn to the healing dynamic of sharing with others experiencing a similar loss, and meaningful materials to read.

Spiritual Counselors

Sometimes the greatest need of a hospice patient or family member is quiet time with our chaplain who can offer a faith-based compassionate listening ear, ease fears, and share messages of hope and peace. Chaplains can also assist in locating a cleric that most closely aligns with the faith of the patient or family and will support those clerics in their care of our patients.

Specially Trained Volunteers

Our hospice volunteers offer warm, personalized support to patients and their loved ones, including assistance with errands, companionship and spiritual support, and staying with patients to give primary caregivers time away to accomplish short-term tasks like shopping or attending an event.
