Certified Nursing Assistant (CENA) – Home Care

Shift: Full-time, 80 Hours Per Pay Period, Monday-Friday and weekend rotation.

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to home care patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Registered Nurse (RN) – Home Care

Shift: Full-time, 80 Hours Per Pay Period, Monday-Friday.

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

Intake/Admissions Nurse

Shift: Part-time, 48 Hours Per Pay Period, Monday-Friday and weekend rotation.

The Intake/Admission Nurse is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.

Social Worker

Shift: 80 Hours Per Pay Period, Monday-Friday. Coverage in our Northern Service Area.

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Dietary Aide PRN – Poppen Residence  

Shift: As needed (weekday, evenings and weekends).

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Poppen Residence PRN Certified Nursing Assistant (CENA)            

Shift: 72 Hours Per Pay Period, 6:00 pm – 6:30 am, Monday-Friday and Every Other Weekend

The CENA under the direction and ongoing supervision of the registered nurse, provides direct care to patients in the Poppen Hospice Residence including essential personal care with baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed.

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
