Current Job Postings

Social Worker – Palliative Care                

The Social Worker coordinates services for the palliative care program in community based palliative care setting. Provides supportive counseling, anticipatory guidance and assistance to patients and their families with personal, psychosocial, psychoeducation, and environmental difficulties related to the patient’s illness.

Acts as liaison with area discharge planners, referrers, payers and patients/families through community education, regular contacts and other marketing activities.  Plans and conducts public relations activities designed to create and maintain favorable public image for Harbor Palliative Care.

Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with serious illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual.

Qualifications:

  • Graduate of Master’s program in social work accredited by the Council on Social Work Education and holds current State of Michigan Certification.
  • Current State of Michigan Social Work License (LMSW or LLMSW).
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Certified Nursing Assistant (CENA) – PRN – Poppen Residence                      

Shift: As needed (weekday, evenings and weekends)

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients.

Qualifications:

  • Current state of Michigan Nurse Aide Certification, and is currently listed in good standing on the State nurse aide registry.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Intake/Admission Registered Nurse      

Shift: 80 Hours Per Pay Period, 10:00 am – 8:30 pm, 4 Days/Week and Weekends Vary

The Intake/Admission Nurse is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • BSN preferred with experience in community-based care.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Hospice Registered Nurse OR Licensed Practical Nurse – Home Care RN/LPN     

$2,500 Sign-On Bonus                 

Shift: 38.5 Hours Per Pay Period, M-F 5:00 pm – 9:30 pm, Every Other Weekend 12:00 pm – 8:30 pm

The RN/LPN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

RN PRN Residence Nurse – Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
