Systems Support Specialist

Shift: Full-time, 80 Hours Per Pay Period, 8:00 am-5:00 pm

The Systems Support Specialist assists the Systems Manager to maintain an operational and effective IT system.

Residence Nurse (LPN) – Poppen Residence

Shift: Part-time, 7:00 pm-11:00 pm, flexible schedule

The LPN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board Nursing, the patient, and the policies of Harbor Hospice.

Night Shift Registered Nurse (RN)

$5,000 sign-on bonus – Shift: Full-time, 4:30 pm – 8:00 am

The Night Shift Registered Nurse is responsible for providing nursing care, teaching, and supportive care to terminally ill patients and their families in their home, in accord with Hospice Patient Care Plans. Schedule is seven days per pay period, which includes every other weekend.

Float Registered Nurse (RN) – Home Care/Poppen Residence

Shift: Full-time, flexible schedule

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Float Certified Nursing Assistant (CENA) – Home Care/Poppen Residence

Shift: Full-time, flexible schedule

The CENA under the direction and ongoing supervision of the registered nurse provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Dietary Aide

Shift: Full-time, 40 Hours Per Pay Period, Rotates weekends and holidays

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Intake Registered Nurse (RN)

$5,000 sign-on bonus – Shift: Full-time, 60 Hours Per Pay Period (10-hour shifts), Every Saturday, Sunday, and Monday

The Intake RN is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers, and patients/families.

Certified Nursing Assistant (CENA) – Poppen Residence                      

$2,500 sign-on bonus – Shift: Full-time, 6:00 am – 6:30 pm, and every other weekend.

The CENA under the direction and ongoing supervision of the registered nurse provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Social Worker PRN

Shift: As needed (weekday)

The Social Worker provides support, counseling, and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Dietary Aide PRN – Poppen Residence  

Shift: As needed (weekday, evenings and weekends).

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).
