Social Worker (MSW) – Home Care

Shift: Full-time, 64 Hours Per Pay Period, Monday-Friday

The Social Worker coordinates social work services for the home care program in a community-based care setting. Provides supportive counseling, anticipatory guidance, and assistance to patients and their families with personal, psychosocial, psychoeducation, and environmental difficulties related to the patient’s illness.  

Medical Records Specialist

Shift: Full-time, 64 Hours Per Pay Period, Monday-Friday

The Medical Records Specialist is responsible for maintaining accurate patient charts to comply with state, federal and JCAHO regulations, computer data entry into the patient management system, and word processing.

Clinical Residence Manager – Poppen Residence

Shift: Full-time, 80 Hours Per Pay Period, Monday-Friday, and administrator on-call rotation.

The Residence Manager/Director of Nursing (RM/DON) is responsible for the oversight and provision of the residence services, including the employment of qualified Hospice personnel.  The RM/DON establishes, implements, and evaluates the agency’s care, treatment, and services to comply with federal and state regulations; maintains quality standards, Joint Commission accreditation, and promotes the organization’s philosophy of care.  Responsible for the day-to-day services of the building which includes oversight of housekeeping, maintenance, and grounds. The RM/DON develops strategies for growth and program enhancements in support of the organization’s strategic plan.

The RM/DON leads the Poppen nursing team and creates a warm and supportive culture for our patients, their families, and hospice staff.  The RM/DON administers the nursing program as DON, holds staff accountable to the nursing service objectives, policies, and standards of nursing practice. Manages, directs, and schedules the work of licensed nurses and nurse aides. 

The RM/DON considers the organization’s mission and the organization’s business needs in all decision-making. 

QUALIFICATIONS

  1. Skilled nursing facility management experience and training in gerontology preferred.
  2. 1 Year of hospice experience required.
  3. 2 years of proven management experience preferred.
  4. Current Registered Nurse license required, CHPN or will attain within two years. Bachelor’s degree in Nursing preferred.
  5. Must have excellent communication and interpersonal skills.
  6. Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Registered Nurse (RN) – Poppen Residence

Shift: Full-time, 72 Hours Per Pay Period, 7:00 am – 7:30 pm

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Dietary Aide PRN – Poppen Residence  

Shift: As needed (weekday, evenings and weekends).

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Certified Nursing Assistant (CENA) – Poppen Residence           

Shift: 72 Hours Per Pay Period, 6:00 pm – 6:30 am

The CENA under the direction and ongoing supervision of the registered nurse, provides direct care to patients in the Poppen Hospice Residence including essential personal care with baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed.

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
