Current Job Postings

Intake/Admission RN – Registered Nurse

Shift:  Part-time, 32 Hours Per Pay Period, 8:00 am – 4:30 pm, 2 days a week, weekends vary.

The RN Intake/Admissions is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families. Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed. Assesses patient/family interest in hospice care and services as needed.

Home Care RN Case Manager

Shift: Full-time, 80 Hours Per Pay Period, Mon-Fri 8:00 am – 4:30 pm, weekends vary

The Home Care RN Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Palliative Care RN and Case Manager

Shift Full-time, 72 Hours Per Pay Period, Mon-Fri 8:00 am – 5:00 pm

The Palliative Care Registered Nurse Case Manager coordinates all Interdisciplinary aspects of care in all settings through assessment and reassessment functions. Identifies patient/family needs through the interdisciplinary process.  Collaborates and coordinates care provided by the specific interdisciplinary team.

Quality and Compliance Manager – Registered Nurse

Shift: Full-time, 64 Hours Per Pay Period, 8:00 am – 5:00 pm

The Quality and Compliance Manager plans, designs, implements and maintains the Harbor Hospice Performance Improvement and Compliance Programs. Responsible for maintaining and monitoring quality assurance data and public reporting .  Develops and implements policies, procedures, TQM activities, reporting, training, and education to ensure organization-wide compliance with legal, ethical, contractual, and/or regulatory standards (i.e. CMS, State of Michigan, OIG corporate compliance, the Joint Commission, HIPAA etc).

Coordinates with Clinical Manager the designing and implementing effective staff education and training programs; is able to work well with others; inspires confidence and respect.

Dietary and Housekeeping Aide

Shift: Part-time, 40 Hours Per Pay Period, Every Other Weekend

The Dietary and Housekeeping Aide is responsible for providing dietary and housekeeping support to terminally ill patients and their families in the hospice residence, in accordance with the hospice patient care plans.

Social Worker (LLMSW, LMSW) Transitions Navigator

Shift: Full-time, 80 Hours Per Pay Period

The Social Worker Transitions Navigator facilitates integrated care delivery across the health care continuum to assure seamless transitions for patients who may benefit from hospice and/or palliative care services. Maintains regular communication with area discharge planners, referrers, payers and patients/families through community education, regular telephone or in-person visits and other information sharing activities.

Director of Business Operations

Shift: Full-time, 80 Hours Per Pay Period

The Director of Business Operations will be responsible for the overall operations of the agency’s business and administrative functions, facilities management, financial services, accounting procedures, and is a member of information technology services.

Hospice Registered Nurse (RN) – Home Care

Shift: Part-time, 38.5 Hours Per Pay Period, Mon-Fri 4:30 pm – 9:00 pm, Every Other Weekend 12:00 pm – 8:30 pm

The Home Care RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Certified Nursing Assistant (CENA) – Home Care

Shift: Full-time, 80 Hours Per Pay Period, Mon-Fri and On-Call Weekends

The Home Care CENA provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – Home Care

Shift: Full-time, 64 Hours Per Pay Period, Week Days Vary and Every Other Weekend

The Home Care CENA provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – Home Care

Shift: Part-time, 48 Hours Per Pay Period, Week Days Vary and Every Other Weekend

The Home Care CENA provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – Poppen Residence                      

Shift: Part-time, 1st Shift (12 Hours),  48 Hours Per Pay Period

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Bereavement Counselor – PRN

Shift: 8:00 am – 4:30 pm as needed.

The Bereavement Counselor is responsible to plan and implement interventions to meet the needs of families/caregivers from Harbor Hospice for thirteen months following the death of the individual Hospice patient. The bereavement counselor may also assist in providing grief and loss services provided by hospice to the community-at-large.

Harbor Hospice is located in Muskegon, but we are interested in talking to candidates in Oceana County or Ludington area as well for this position would cover these areas.

Social Worker (MSW) – PRN        

Shift: 8:00 am – 4:30 pm, as needed.

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed.

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
