Clinical Outreach Specialist – Registered Nurse       

Shift: 80 Hours Per Pay Period, M-F

The Clinical Outreach Specialist-Registered Nurse, utilizes clinical expertise to contribute to the development of the agency marketing plan and coordinates community education/outreach opportunities including clinical aspects for hospice and palliative care in a manner consistent with the philosophy and policy of Harbor Hospice.

This position plans and coordinates marketing and education initiatives, speaking opportunities along with assigning responsibility to team members as deemed appropriate.  Individual is liaison to identified hospitals, facilities, community partners, physicians, medical health professionals, payers and patients/families and all other referral sources through distribution of educational material, in-services, conferences, regular contacts and other marketing activities.

Qualifications:

  • Bachelor’s degree (B.A.) in nursing from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Registered Nurse – Intake/Admissions      

Shift: 40 Hours Per Pay Period, 10:00 am – 6:30 pm, weekends and holidays may vary

The RN Intake/Admissions is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.  Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed.  Assesses patient/family interest in hospice care and services as needed.

Qualifications:

  • A graduate of an accredited School of Nursing.  BSN preferred with experience in community based care.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Registered Nurse Case Manager – Home Care RN          

Shift: 80 Hours Per Pay Period, M-F 8:00 am – 4:30 pm, weekends vary

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Spiritual Care Coordinator – Home Care              

Shift: 80 Hours Per Pay Period

The Spiritual Care Coordinator is responsible for the provision of spiritual care services to patients/families/caregivers of Harbor Hospice, either directly or through coordination of care with spiritual care counselors

Qualifications:

  • Graduate of accredited seminary or school of theology or Clinical Pastoral Certificate CPE (B.D., M.Div. or equivalent theological degree).
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Referral Liaison / Part-time          

Shift: 40 Hours Per Pay Period, and every third weekend

The Referral Liaison assists the intake department in maintaining the smooth operations of the admissions functions of the agency.

Qualifications:

  • Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • Maintains a valid Michigan driver’s license.

Hospice Registered Nurse OR Licensed Practical Nurse – Home Care RN/LPN     

$2,500 Sign-On Bonus                 

Shift: 38.5 Hours Per Pay Period, M-F 5:00 pm – 9:30 pm, Every Other Weekend 12:00 pm – 8:30 pm

The RN/LPN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Certified Nursing Assistant (CENA) – PRN – Poppen Residence                      

Shift: As needed (weekday, evenings and weekends)

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

RN PRN Residence Nurse – Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Qualifications:

  • A graduate of an accredited School of Nursing.
