Spiritual Care Counselor

Shift: Full-time, 64 Hours Per Pay Period

The Spiritual Care Counselor is responsible for the provision of spiritual care services to patients/families/caregivers of Harbor Hospice.

Hospice Social Worker (MSW)

Shift: Full-time, 80 Hours Per Pay Period

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Dietary Services Coordinator – Poppen Hospice Residence

Shift: Full-time, 80 Hours Per Pay Period

The Dietary Services Coordinator is a supervisory position responsible for providing and overseeing dietary and housekeeping services at the Leila & Cyrus Poppen Hospice Residence.

This position is responsible for supervising the maintenance and housekeeping staff in the residence to ensuring all systems are in compliance with applicable safety and fire regulation and federal, state and local building codes to ensure a safe and comfortable environment.

President/CEO

Shift: Full-time, 80 Hours Per Pay Period

The President/CEO will assume responsibility and accountability for overall operations of the Harbor Hospice organization.  This individual is responsible for the development, implementation and evaluation of the organization’s programs and services to meet its mission.

QUALIFICATIONS

Master’s degree in Health Care, Public Health, Business, and Social Work preferred. Bachelor’s degree is required.

The CEO/President is the resident agent of the organization and has responsibility to monitor the organization’s compliance with laws, regulations and standards of care under which Harbor Hospice operates.

Qualifications:

Intake/Admissions Registered Nurse RN

Shift: Full-time, 80 Hours Per Pay Period

The Intake/Admissions RN is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.

Dietary and Housekeeping Aide

Shift: Part-time, 40 Hours Per Pay Period, Every Other Weekend

The Dietary and Housekeeping Aide is responsible for providing dietary and housekeeping support to terminally ill patients and their families in the hospice residence, in accordance with the hospice patient care plans.

Social Worker (LLMSW, LMSW) Transitions Navigator

Shift: Full-time, 80 Hours Per Pay Period

The Social Worker Transitions Navigator facilitates integrated care delivery across the health care continuum to assure seamless transitions for patients who may benefit from hospice and/or palliative care services. Maintains regular communication with area discharge planners, referrers, payers and patients/families through community education, regular telephone or in-person visits and other information sharing activities.

Residence Manager/Director of Nursing

Shift: Full-time, 8:00 am – 4:30 pm, Administrator on call rotation

The Residence Manager/Director of Nursing is responsible for the oversight and provision of the residence services, including the employment of qualified Hospice personnel.  The RM/DON establishes, implements, and evaluates the agency’s care, treatment, and services to comply with federal and state regulations; maintains quality standards, Joint Commission accreditation, and promotes the organization’s philosophy of care.  Responsible for the day-to-day services of the building which includes oversight of housekeeping, maintenance, and grounds.

Director of Business Operations

Shift: Full-time, 80 Hours Per Pay Period

The Director of Business Operations will be responsible for the overall operations of the agency’s business and administrative functions, facilities management, financial services, accounting procedures, and is a member of information technology services.

Hospice Registered Nurse (RN) – Home Care

Shift: Part-time, 38.5 Hours Per Pay Period, Mon-Fri 4:30 pm – 9:00 pm, Every Other Weekend 12:00 pm – 8:30 pm

The Home Care RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

Bereavement Counselor – PRN

Shift: 8:00 am – 4:30 pm as needed.

The Bereavement Counselor is responsible to plan and implement interventions to meet the needs of families/caregivers from Harbor Hospice for thirteen months following the death of the individual Hospice patient. The bereavement counselor may also assist in providing grief and loss services provided by hospice to the community-at-large.

Certified Nursing Assistant (CENA) – Home Care

Shift: Part-time, 48 Hours Per Pay Period, Week Days Vary and Every Other Weekend

The Home Care CENA provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Social Worker (MSW) – Palliative Care and Advance Care Planning

Shift: Full-time, 64 Hours Per Pay Period

The Palliative Care Social Worker conducts psycho-social assessments and coordinates supportive services for patients and their families who are facing serious illness in a variety of settings. Provides counseling, anticipatory guidance and assistance to patients and their families with personal, psycho-social, psycho-education, and environmental difficulties related to the patient’s illness and goals of care as a member of the Palliative Care Interdisciplinary team. Educates patients and the community regarding Advanced Care Planning and assists with completion of advanced care directives. Collects and maintains data regarding Advance Care Planning initiatives and community impact.

Social Worker (MSW) – PRN        

Shift: 8:00 am – 4:30 pm, as needed.

The Social Worker provides support, counseling and assistance to patients and their families with personal, psychosocial, and environmental difficulties related to the patient’s terminal illness.

Shift: Part-time, 1st Shift (12 Hours),  32 Hours Per Pay Period

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: 7:00 pm – 7:00 am, as needed.

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
