Social Worker (MSW) – Home Care – Northern Area

Shift: Full-time, 80 Hours Per Pay Period, Dayshift-weekdays

The Hospice Social Worker coordinates social work services for the home care program in community based care setting. Provides supportive counseling, anticipatory guidance and assistance to patients and their families with personal, psychosocial, psychoeducation, and environmental difficulties related to the patient’s illness.

Registered Nurse (RN) Case Manager – Home Care

Shift: Full-time, 80 Hours Per Pay Period, Dayshift-weekdays, On-call weekend rotation

The Hospice Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice. Hospice Registered Nurse creates & oversees plans of care for each patient admitted to Home Care.

Hospice Registered Nurse (RN) – Home Care

Shift: Part-time, 48 Hours Per Pay Period, Dayshift-weekdays

The Hospice Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice. Hospice Registered Nurse creates & oversees plans of care for each patient admitted to Home Care.

Grief Support Counselor

Shift: Full-time, 64 Hours Per Pay Period, Dayshift-weekdays

The Grief Support Counselor is responsible to plan and implement interventions to meet the needs of families/caregivers from Harbor Hospice for thirteen months following the death of the individual Hospice patient.  The bereavement counselor may also assist in providing grief and loss services provided by hospice to the community-at-large for all age populations, including children.

Intake Registered Nurse (RN)

Shift: Full-time, 60 Hours Per Pay Period (10-hour shifts), rotating weekends.

The Intake RN is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers, and patients/families.

Dietary Aide PRN – Poppen Residence  

Shift: As needed (weekday, evenings and weekends).

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Registered Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: As needed

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
