Clinical Education Coordinator

Shift: Full-time, Mon-Fri 8:00 am – 5:00 pm, and as needed.

The Clinical Education Coordinator provides educational support by ensuring clinical staff meet regulatory requirements for licensure and encourages continuing education for all staff. Participates in the development and monitoring of competency skills including Electronic Medical Record (EMR)for assigned staff members.

Coordinates with clinical staff as necessary to assure quality standards are met in compliance with Standards of Practice and agency policy.

Provides education and technical support to patient care staff to facilitate meeting quality standards in compliance with Medicare and Joint Commission Hospice standards. Develops and implements annual Skills Fair for Clinical Staff (Nurse and CENAs).

QUALIFICATIONS:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Proficient in Clinical Technology.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Executive Assistant

Shift: Full-time, Mon-Fri 8:00 am – 5:00 pm, and as needed.

The Executive Assistant coordinates and assists the Executive Director in maintaining the smooth operations of the executive and development functions of the agency.

Provides administrative support for the President & CEO and the Senior Leadership Team to coordinate activities and meetings of the Board of Directors and its committees, ensuring timely notice and information sharing with the Board.  Includes preparation of all memorandums, agendas, packets, minutes and conference calls.

Coordinates the scheduling of and facility arrangements for meetings of the President & CEO and Senior Leadership Team as needed.

Coordinate executive communications, including taking calls, responding to emails and interfacing with leadership, management, and staff.

Plans/organizes and implements events such as meetings, business luncheons, client dinners and staff appreciation events, both within the organization and outside of the work place.

The ideal candidate has experience in an active and complex executive support role.

This position will require you to work independently while juggling multiple projects with tight deadlines—excellent time management and organizational skills are necessary.

QUALIFICATIONS:

  • Bachelor’s degree (B.A.) from a four-year college or university; or one to two years of experience as an Executive Assistant reporting directly to senior management.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Clinical Outreach Specialist – Registered Nurse RN   

Shift: Full-time, Mon-Fri 8:00 am – 5:00 pm, and as needed.

The Clinical Outreach Specialist utilizes clinical expertise to contribute to the development of the agency marketing plan and coordinates community education/outreach opportunities including clinical aspects for hospice and palliative care in a manner consistent with the philosophy and policy of Harbor Hospice.

This position plans and coordinates marketing and education initiatives, speaking opportunities along with assigning responsibility to team members as deemed appropriate.  This position acts as a liaison to identified hospitals, facilities, community partners, physicians, medical health professionals, payers and patients/families and all other referral sources through distribution of educational material, in-services, conferences, regular contacts and other marketing activities.  Creates and maintains a favorable public image for Harbor Hospice and Palliative Care and raises community awareness regarding those facing serious illness and end of life choices.

QUALIFICATIONS:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.

Registered Nurse Case Manager – Home Care RN                       

Shift: 72 Hours Per Pay Period, Mon-Fri 8:00 am – 4:30 pm, weekends vary

This is a full-time position that is benefit eligible.

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.

QUALIFICATIONS:

  • A graduate of an accredited School of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Dietary/Housekeeping Aide    

Shift: 40 Hours Per Pay Period, Part-time, weekly schedule and weekends vary.

The Dietary/Housekeeping Aide is responsible for providing dietary support to terminally ill patients and their families in accordance with Hospice Patient Care Plans. Cooks meals in quantities according to menu. This position will also perform light housekeeping duties using safe procedures with includes knowledge and use of chemicals.

QUALIFICATIONS:

  • The employee is occasionally required to stand and walk.
  • The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds

Certified Nursing Assistant (CENA) – Poppen Residence                      

Shift: 48 Hours Per Pay Period, 6:00 am – 2:30 pm, every other weekend.

The CENA under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Qualifications:

  • A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  • Current State of Michigan Nurse Aide Certification
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Shift: 72 Hours Per Pay Period, 2:00 pm – 10:30 pm, every other weekend
