Night Shift Registered Nurse (RN)

Shift: Full-time, 4:30 pm – 8:00 am

The Night Shift Registered Nurse is responsible for providing nursing care, teaching, and supportive care to terminally ill patients and their families in their home, in accord with Hospice Patient Care Plans. Schedule is seven days per pay period, which includes every other weekend.

Community Marketing Liaison

Shift: Full-time

The Community Marketing Liaison contributes to the development of the agency marketing plan and coordinates community education/outreach opportunities for programs of Harbor Hospice in a manner consistent with the philosophy and policy of the organization.

Case Manager – Home Care (RN)

Shift: Full-time, 80 Hours Per Pay Period, Monday-Friday, 8:00 am – 4:30 pm with on-call weekend rotations.

The Hospice Nurse Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice. Creates & oversees plans of care for each patient admitted to the Home Care Harbor Hospice program.

Bereavement Counselor

Shift: Full-time, 64 Hours Per Pay Period, Monday-Friday

The Bereavement Counselor is responsible to plan and implement interventions to meet the needs of families/caregivers from Harbor Hospice for thirteen months following the death of the individual Hospice patient.  The bereavement counselor may also assist in providing grief and loss services provided by hospice to the community-at-large for all age populations, including children.

Intake Registered Nurse (RN)

Shift: Full-time, 60 Hours Per Pay Period (10-hour shifts), Every Friday, Saturday, and Sunday

The Intake RN is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers, and patients/families.

Executive Assistant

Shift: Full-time, 80 Hours Per Pay Period, Monday-Friday

The Executive Assistant coordinates and assists the President/CEO in maintaining the smooth operations of the executive and development functions of the agency.

Provides administrative support for the President & CEO and the Senior Leadership Team to coordinate activities and meetings of the Board of Directors and its committees, ensuring timely notice and information sharing with the Board.

Coordinates the scheduling of and facility arrangements for meetings of the President & CEO and development departments.

Coordinate executive communications, including taking calls, responding to emails, and interfacing with leadership, management, and staff.

Plans/organizes and implements events such as meetings, business luncheons, client dinners, and staff appreciation events, both within the organization and outside of the workplace.

The ideal candidate has experience in an active and complex executive support role.

This position will require you to work independently while juggling multiple projects with tight deadlines—excellent time management and organizational skills are necessary. 

Dietary Aide PRN – Poppen Residence  

Shift: As needed (weekday, evenings and weekends).

The Dietary Aide is responsible for providing dietary support to terminally ill patients and their families in the hospice residence, in accord with Hospice Patient Care Plans.

Certified Nursing Assistant (CENA) – Home Care  

Shift: Full-time, 80 Hours Per Pay Period

The CENA under the direction and ongoing supervision of the registered nurse provides direct care to patients in the Poppen Hospice Residence including essential personal care with baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – Poppen Residence           

Shift: Full-time, 72 Hours Per Pay Period, 6:00 am – 6:30 pm

The CENA under the direction and ongoing supervision of the registered nurse provides direct care to patients in the Poppen Hospice Residence including essential personal care with baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Certified Nursing Assistant (CENA) – PRN – Home Care/Poppen Residence                      

Shift: As needed (weekday, evenings and weekends).

The CENA under the direction and ongoing supervision of the registered nurse provides essential personal care assistance to patients including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.

Residence Nurse (RN) – PRN – Home Care/Poppen Residence                       

Shift: As needed

The Registered Nurse provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient, and the policies of Harbor Hospice.
